Glad Lash Trainers, please complete the following form to request certificates for students who have completed training. Note!

• 1 student per form.
• Please double check all information is correct before submitting.
• Course completion is not achieved until students submit the required photos of their work. Please ensure this has been completed before submitting certificate requests.
• Certificates will be emailed to trainers to pass on to students.
• Certificates will be emailed within 24 hours or the next business day.
